• Open Positions (Case Managers) in NYC Permanent Scattered Housing Site Program
• Flexible schedule, Mon-Friday, 1 day remote,
• BA/BS degree or 1-2 yrs. of exp.
• Salary up to $48,000
• Locations: Bronx & Manhattan Shelters

Duties:

• Manage caseload of approximately 25.
• Conduct intake and psychosocial and substance abuse assessments of clients.
• Prepares and monitors the Individual Case Management Plan for each client.
• Identify short and short-term objectives to be achieved.
• Inputs client information and updates into the CARES system.
